Social Prescribing

What is this all about? 

The social prescribing and health and wellbeing service is an exciting new NHS service that is now embedded into local Primary Care Networks to support and maintain our local population’s health.  

Social Prescribers and our Health & Wellbeing Coach can refer, connect and signpost people to a wide range of local services, specialist groups, local council, voluntary groups, support groups and activities relevant to them to help improve their health and wellbeing. This can be in the form of financial advice, mental health support,  housing, lifestyle changes, exercise groups, and more.  

Who is it aimed at?

The service is aimed at all patient groups including carers, for financial or debt problems, employment issues, family issues, homelessness, loneliness, getting older, single parents, bereavement, exercise, weight management, and much more.

What do they do?

Social Prescribers are able to spend time with the patient, to listen, understand and create a personalised support plan. This shifts the focus from “What’s the matter with you” to “What matters to you” and often leads to positive long term wellbeing benefits.

The health & wellbeing coaching service can support patients with everything from improving sleep, managing stress & low mood, becoming more active, losing weight, newly diagnosed with type 2 diabetes or pre-diabetes, better managing their long-term health condition with diet or lifestyle changes, to name a few. 

How do I get referred? 

Your GP may suggest a referral or you can contact our reception team to ask them to refer you.


Unfortunately, we can't accept a referral from patients with the following: 

 Under 16' s
 Patients in Palliative Care
 Any type of medical advice or urgent care.
 Severe mental health conditions
