Submit an online request
What you can do online:
- request a Routine GP appointment
- request an Urgent 'on the day' GP appointment (you will be contacted same day, please specify your availability, am / pm, etc) (If the health issue is life threatening you must contact 999 immediately - do not submit an online form).
- submit administration queries, like fit (sick) notes, test results, repeat prescriptions, queries for the Secretaries, etc
Please ensure you include all relevant information on the online form, including your current contact details. If the contact details on your medical records do not match, you can update your contact details.
If you are struggling to fill out the form, please call the Surgery (01789 292895). Please note that to keep our phone lines clear for urgent enquiries, a member of our admin team will call you back to assist you in completing the form.
All Practice Nurse appointments continue to be bookable by calling the Surgery on 01789 292895, and don't forget that you can self-refer to a variety of health services without seeing your GP.

Multiple Problems Per Consultation Policy
Please understand that the doctor may ask you to rebook or submit a further online request if you are presenting with more than one issue.
We know that getting an appointment can sometimes feel difficult, but we are confident that our new doctor-led triage system has made it easier for our patients to gain access to our services and will feel confident when they contact us that they will be offered an appropriate appointment based on their needs.
Here are some tips to make the most of your appointment.
- Deal with the most pressing issue you have first
- If it’s your first visit to the surgery, bring a list of medications
- If you don’t understand something, please ask the doctor to explain
- If it helps you, ask a friend or family member to come with you
- Make sure you know what happens next, you may need tests or a follow-up appointment
- Patients with dental issues need to see a Dentist – if you do not have a dentist please call 111
If you are more than 10 minutes late for your appointment – your clinician may not be able to see you and you may be asked to rebook.
Zero Tolerance – respect all staff and other patients visiting the Surgery – disruptive or abusive behaviour will not be tolerated and you will be asked to leave.
