ADHD / Autism Referrals (Adults and Children) – Warwickshire
What is ADHD?
Attention Deficit Hyperactivity Disorder (ADHD) is a condition that can affect attention, activity levels, and impulse control. It can affect both children and adults and may impact school, work, and daily life.
Who can be referred?
We can refer children, young people, and adults. The main factor is whether ADHD or autism-related difficulties have a moderate impact on daily life – for example, at work, school, home, or in relationships. You do not need to prove the diagnosis yourself; we just need to understand how things are affecting you.

When to Seek Help
You may wish to seek advice if your child has ongoing difficulties such as:
- Trouble concentrating or staying focused
- Being very active or restless
- Acting impulsively
- Struggling at school or at home
How to Request an Assessment
- Through School (Recommended)
- Speak to your child’s teacher or Special Educational Needs Co-ordinator (SENCo)
- The school can gather information from both home and classroom settings
- With your consent, they can refer to specialist services

- Through Your GP
- Book an appointment to discuss your concerns
- You will be asked to complete behaviour questionnaires
- You will be asked to provide an impact assessment / statement
- If appropriate, your GP can refer your child for assessment

Important Information
- Parents and carers cannot self‑refer directly to NHS ADHD services
- Referrals must come from a GP, school, or other professional
- Referrals are reviewed against clinical criteria before assessment
- Right to choose referrals can only be made through the GP

When to Seek Help
You may wish to speak to a GP if you have:
- Ongoing difficulties with attention or organisation
- Restlessness or impulsivity
- Problems managing work, relationships, or daily tasks
How to Request an Assessment
Through Your GP (Main Route)
- Submit an online consultation to request an appointment to discuss your symptoms
- You will be asked to complete a screening questionnaire and impact assessment ahead of the appointment to discuss your symptoms further (see below for questionnaire links).
- Your GP will decide if a referral is appropriate

Right to Choose (Alternative NHS Providers)
You may choose an alternative NHS‑funded provider through the Right to Choose scheme. Your GP can advise if this option is suitable for you.

RIGHT TO CHOOSE ADHD/ASD
In England, patients have the option of NHS Right to Choose. This allows patients to access the shortest waiting times within the NHS.
Below you will find the relevant information on current NHS Right to Choose providers, the information required from the patient by your GP Surgery and information relating to our Practice Shared Care Policy.
Please bring written impact statement and relevant completed questionnaires to reception for the attention of your doctor and they will contact you should any further information be needed ahead of processing your referral.
Important to note : If there is any school involvement, please could we also have a copy of any school letters and/or ECHP’s which are in place.

What if I already have aN ADHD diagnosis and have declined medication at the time?
If you have a previous diagnosis, and declined medication at the time, you will need to go back to the ADHD consultant / provider where you had your assessment and formal diagnosis as they will either:
Reaccept you on to their patient list and reassess your management plan. Please note that there may be a waitlist for this.
Ask for a GP referral to be sent to them for you so that they can accept you back onto their patient list. Please note that there may be a waitlist for this.
They will say you have been discharged and cannot be accepted back as a patient. In this case most clinics will not accept you as a patient as you already have a formal diagnosis. This means that you may have to go privately to pursue medication options which your GP may not be able to continue prescribing.
Occasionally NHS right to choose clinics may accept patients but again we advise you to contact the clinics directly to see if you would be accepted with an NHS referral from the GP
