GP APPOINTMENTS
The Surgery runs a Doctor-Led Total Triage System for all our Doctor's appointments. This requires the patient (their legal proxy representative, or the receptionist if patients need help) to complete an online submission between 7.30am and 6.30pm Monday to Friday. The submission is then presented electronically to the Doctor-Led Total Triage Working Team who will review all requests, both medical and administrative.
One Problem Per Consultation Policy
Please understand that the doctor may ask you to rebook or submit a further online request if you are presenting with more than the one issue that you initially contacted us about.
We know that getting an appointment can sometimes feel difficult, but we are confident that our new doctor-led triage system has made it easier for our patients to gain access to our services and will feel confident when they contact us that they will be offered an appropriate appointment based on their needs.
Here are some tips to make the most of your appointment.
• Deal with the most pressing issue you have first
• If it’s your first visit to the surgery, bring a list of medications
• If you don’t understand something, please ask the doctor to explain
• If it helps you, ask a friend or family member to come with you
• Make sure you know what happens next, you may need tests or a follow up appointment
• Patients with dental issues need to see a Dentist – if you do not have a dentist please call 111
If you are more than 10 minutes late for your appointment – your clinician may not be able to see you and you may be asked to rebook.
Zero Tolerance – respect all staff and other patients visiting the Surgery – disruptive or abusive behaviour will not be tolerated and you will be asked to leave.

The GP's dealing with the requests for medical appointments will triage based on medical urgency.
GP's will signpost patients to the most appropriate service at the most appropriate time, this may include signposting to the Government backed Pharmacy 1st Service for minor ailments which the Pharmacy can deal with and, in some instances, prescribe for. Also to the Minor Injuries Unit at both Warwick Hospital and Stratford Hospital and if appropriate to other care providers, including the Community Nursing Team.
Links to book the GP appointments will be sent to the patients via a text response to their online submission. When a patient requests a call rather than a text then an Administrator will contact the patient to offer an appointment as suggested by the GP.
Please ensure the forms are completed as fully as possible indicating the reasons for your requested appointment and that the contact details we hold for you are up to date. The Reception Team will continue to provide telephone support to assist patients in completing online submissions.
If you have a life threatening emergency (this includes any chest pain) always call 999. If you need advice or feel you need to see the GP after the Total Triage ceases for the day please call the Surgery on 01789 292895 and the Reception Team will refer your query to the Total Triage Team for their advice.
Home Visits
Home Visits are offered to house bound patients only. When contacting the Practice via online submission to request a home visit please do so before 10.30am in order that visits can be planned for the afternoon.
NURSing & PHLEBOTOMY (bLOOD TEST) APPOINTMENTS
Patients should continue to call the Surgery on the main number 01789 292895 to book appointments with the Nurses/Healthcare Assistants and Phlebotomist. Some of these appointments will also be available to book via the NHS App.

ADMINISTRATION REQUESTS
Administration requests for continuation Fit Notes, GP letters, recent test results, etc should all be submitted via online submission. Please note however that a request for a first time Fit Note will only be issued after the patient has completed 7 days of self-certification and then only after having seen or spoken to a GP in an appointment.
When THE PRACTICE IS Closed
When the practice is closed, you can call the Surgery for further instructions, call NHS 111 for medical advice on urgent matters or use NHS111 online
NHS 111 can help if you have an urgent medical problem and you’re not sure what to do. NHS 111 is available 24 hours a day, 7 days a week and you can get help online or on the phone.
If you have difficulties communicating or hearing, you can:
- call 18001 111 on a textphone
- use the NHS 111 British Sign Language (BSL) interpreter service if you’re deaf and want to use the phone service
How it Works
You answer questions about your symptoms on the website, or by speaking to a fully trained adviser on the phone.
You can ask for a translator if you need one.
Depending on the situation you’ll:
