Register as new patient
We welcome new patients who live within our practice area to register with us see Practice Catchment Area here.
Please check our practice boundary by submitting your post code into the search bar. Please note that Wellesbourne, Ettington, and some areas of Warwick are outside our boundary.
Preferences
Patients have the right to express a preference from whom they receive services. This preference will be recorded and the practice will endeavour to comply with any reasonable preference expressed but need not do so if the person requested has reasonable grounds for refusing to provide the services or does not routinely provide the service requested.
Medication
Before you register please ensure you have 1 months supply of medication from your current GP Practice.
Once registered you will need to book a "new patient medication review" to ensure continuity.
Armed Services
Patients registering with us who have been registered with the Armed Services please ensure you have your FMed133 care summary in order for us to upload to your NHS record.
In order to obtain your full record from the Ministry of Defence we will require you to complete the attached form and return to us at your earliest convenience. Thank you.
