Update My Details
Please use the form below to update the Practice of any changes to your personal details. If you are submitting updated details for multiple people in your household, you will need to submit a new form for each member of your family.
If you have moved house, please check your new address is still within the Practice catchment area. Patients moving out of the Practice catchment area will need to register with a more local surgery to them - sorry, there is no exceptions. (30 days re-registration time is provided to ensure sufficient time for ordering of repeat medications and transfer of records).
If you are changing your name please attach a copy of the certified document. If you are not able to do this please attend the Surgery IN PERSON, complete a change of name form and show a copy of the certified document (ie. marriage document, decree nisi, deedpoll) to the Reception Team at Trinity Court or Claverdon Surgery. Your name will not be changed until the appropriate documentation has been seen.
